Ed Greenwood is sometimes jokingly accused of inserting his fetishes into the Forgotten Realms setting and disguising them, namely transformation and gender-bending.
Some examples: The Drow Goddess Eilistraee, whose male followers could be transformed into females via ritual and to spend some time as a chick instead of a dude. They could also opt to not transform back into a dude. Elminster, who either spent some time transformed into a girl or transformed himself into a girl to evade notice or because fuck you he's a wizard. The Drow Goddess Lloth, who transforms Drow who have angered her into sub-humanoid Driders as a form of punishment. Various cursed magical items that transform their wearer into the opposite sex.
If there was ever a person who inserted their magical realm into a tabletop game, Ed Greenwood is by far the most successful and widespread of them.

Plus the Girdle of Masculinity/Femininity first appeared in the A&D DMG so that was also Gygax. That being said Greenwood probably got a lot of millage out of it.

Oh, here's one I forgot: every time you see the word "festhall" in a published Forgotten Realms product, that's a place where Mr. Greenwood wrote "brothel." :D
