You play as the girl in the chair, and you have to guess whether the next card drawn is going to be higher, lower, or the same as the last number revealed. High and low nab you a small amount of money, while correctly guessing the exact same number gets you a lot more. If you can make it through the entire deck you can keep the winnings, but only if you pass a certain threshold, meaning that you have to do more than just play it safe the whole way through and use knowledge of the numbers left in the deck to make risks based on statistical probabilities.

Oh, and also every time you guess wrong the guy with the gun *might* blow your brains out. That's a fairly important detail.
