It looks like the brother and sister actually have the Westermarck effect in full effect, but being the self-aware losers they are, just brute-forced their way through their inherent sexual aversion to one another for the sake of convenience. AFAIK the Westermarck Effect only applies when the concerned parties are raised together for the earliest stages of life and ends around age six. I mean, if the protag and childhood friend are the same age and lived next door since birth (or on a commune or if one of them was adopted by the other's family as an infant/toddler) then it would probably kick in but I think the reason the childhood friend loses so damn much is a statement on taking things for granted. When fate drops a princess/MPDG/goddess/whatever into the protag's lap that's basically the universe getting his attention. It doesn't matter that the childhood friend is kind and devoted and in love with him without realizing it - she snoozed and lost because she took protag-kun for granted or was waiting for him to take the initiative. Another thing to consider is that the people who write those kinds of scenarios mindfully (not just lazily following the trope) might have some emotional baggage regarding teenage girls in general and childhood friends in particular.

IMHO, at least some of that would just be the general awkwardness that comes along with being too familiar with someone in a platonic fashion, rather than the Westermarck effect. If someone is a very close childhood friend, you've already got a preconception of their tastes, their positive and negative qualities as an individual, and most likely their romantic preferences as well. And how you measure to those expectations, vice versa. At least from my experiences, you've got little of the intrigue or honeymooning of dating someone less familiar, you've been conditioned not to view them sexually foremost, and there's an existing relationship at risk if you go forward with it. All of that is going to work against a romantic relationship forming, though not prevent it.

In adult life, I'd compare it to relationships in the workplace, though someone in highschool would probably have more apprehension/nervousness about asking someone out regardless, and there's a more practical risk in a rejection at the workplace.

Also, I don't think most romcoms usually portray romance realistically regardless, but that's another can of worms.
