Well, a few things. Presumably.
1) The dream, the fiction dream, is having multiple ladies and not having to worry about them getting their strange on elsewhere. Tiresome life as a harem protagonist that actually fucks, or somesuch.

2) Based on her characterization (what little there is of it), she seems the type to pull a fullbreak as opposed to casually cheating. Y'know, the ol' "I can never be sexual with the person I allegedly love ever again because I've been sullied (and just to drive the point home I'm calling the individual/group that sullied me my 'masters' and regularly meeting up with them when I claim to be doing other shit), this, of course, being based on her being unaware of infidelity. If she were aware, it would be spitecheating, wherein the character involved may or may not have some mid-coitus out-of-body experience realizing "Wait, what the fuck am I doing" and go from there, usually doubling down.

3) Based on the type of cheating Shinozuka Yuuji writes, (checking what's here, yup) the theme seems to follow that the other party never figures it out . While she wouldn't be 'wrong' for doing so (at least, no more wrong than Fujimaki), having it follow her catching him seems to be a line not yet crossed. Heck, based on her thoughts that last page, wouldn't be surprised if she's into it and Kawai's becomes some sort of House of Sin (tm).

All of the above aside, to some extent (discounting those last pages where she wants to "get fucked as a wife") he basically trips into these situations, and if she's prone to the same, then they're really not into "being in a relationship" as much as they're just claiming that they're in one.

Eh, its debatable whether he actually cares about the girlfriend at all really.
