Then, just like the start of the manga, they have an episode and become a public nuisance, gets arrested and without proper representation gets fast tracked through the legal system.

The quality of the prisons and their attached psych ward vary by state but tends to be marginally less nightmarish then depicted, but still fails to provide for the medication, treatment, and environment needed for rehabilitation. In privitized prisons where prisoners are cattle, psych cases just spend their entire time in solitary.

@Hakrei Schizophrenia isn't something that presents as a child, and in women tends to present in their late 20's to early 30's. (In men, it typically presents in their late teens to early 20's.) It can occur quite suddenly, with little to no warning signs. Late onset schizophrenia is even diagnosed in people who are over 45. So, it's quite possible that anyone could have this looming in their future, just waiting for the right trigger to cause a psychotic break.
