Because they don't have to. Futa doesn't have to have balls when the entire concept is fiction, so any notion of where sperm (cum) comes from is pure fantasy.

I'll never understand why artists call their work "futanari" when it has a dick and balls but no vagina; that's shemale/transgender, not futa because it lacks the biological part that differentiates a male from a female.

Here we can see a person who thinks futanari was designed to cater their own definitions and nothing was ever made before they acknowledged the existance of the concept and yet fails to understand it entirely. Futanari means that the host has a vagina and a penis, balls optional. It's the very definition of the fictional concept as old as the manga suggests, I can remember Spunky Knight (1996) buying a damascus sword from with a double-dicked futanari elf with sex and Secret Plot (1995-1997) with the then acceptable term "hermaphrodite" in another story.

I'm sure there are even older works but after "western" artists began to sell comissions from people that don't know better, we get this kind of ignorance.
Please go inform yourself about the damn thing before spouting shit. This manga is from 2001, same year that Bible Black (after the eponymous game) was launched and everybody and their uncle knew about Reika Kitami and her dick without balls and the very first scene is her fucking a student.
