@Cisla: those are a real thing yes. They got popular recently.
In any case, the answer to how they hold still is easy: they eventually don't. This applies to any chastity device that is smaller than your erect size by the way.
If there is nothing else other than your own body holding it, the ring around your ballsack is just not enough. Your body at first is not used to it and your balls will hold your erection, but eventually you will just have erections push the whole ring with the balls outwards and only the tip of the penis will actually be inside the cage.
A solution to this, pretty commonly sold with those micro devices, is an urethral tube or a piercing. That of course, come with the complication of making contracting urinary tract infections way easier.
