On the contrary, it's because Amanoja always writes his shemales as essentially men who know they're men and are just indulging in a fetish that makes them aroused is what makes his stories so uniquely appealing. You can't get this anywhere else.

Amanoja has some stories where a character identifies as a normal man just indulging in surgery for fetish. That’s not the case for all his characters who say they’re men though. I think, in hentai at least (don’t know if the same is true for real people in Japan), trans characters are more likely to make a point of their biological sex even if they consciously live as the other, as lines like this tend to show up a lot in the genre, not just in amanoja’s stuff. Here, I’m pretty sure the character is a normal ‘newhalf’ and not one of Amanoja’s characters with a surgery fetish as she uses a rather feminine speech pattern in the raw with the feminine ‘atashi’ for her personal identifier. For contrast, in the story from page 85 in the raw anthology this came from both characters use the masculine ‘boku’ and outright state they’re not newhalves but just have a fetish.

to my knowledge, in Japanese stating 'I'm a man' like this is a reference to biological sex. I am not an expert, but I know that Japan has very different ideas surrounding gender identity than we do, and I think it involves a stronger decoupling of the physical sex you're born with vs. your personal identity/how you view yourself. If anyone knows more about this than I do, please feel free to correct or elaborate, but my understanding is that you can think of yourself as a woman, feel like a woman, dress and act like a woman, and be accepted as a woman by society, but for the purposes of sexual intercourse and defining yourself in more literal terms, you'd still call yourself a 'man' just 'cause that's what you were born with and that never changes.

Again, not an expert by any means, but that's the understanding I've gleaned from trying to comprehend this stuff when it crops up.

What Mr. 47 describes is very American thinking. It's not common in the rest of the world. Not in Europe, certainly not in East Asia, and ABSOLUTELY not in Central Asia, Africa, or the Middle East.

In fact, even the way Mr. 47 expresses his indignation/disbelief is very American, bordering on ethnocentric.

Newsflash: there's a much larger world outside of American identity politics. Most people, including most Americans, still believe transsexualism to be either a sexual fetish (paraphilia) or a way for flamboyantly gay men to attract not-quite-as-gay men. Hell, I'll even say that most American transsexuals still believe it, deep down in places they don't talk about at parties.
