Yes, this is a whole mess of story telling but I'll try breaking it down

MC Tarou and Izumi (former tomboy) began dating.
Due to her moving to another city they had to settle for a long distance relationship.
She works partime, is uncomfortable with her coworker Kuroda, she always keeps in contact with Tarou and tells him she got invited to an afterwork party.
Kuroda RAPES Izumi AND kept a video out of it to blackmail her. She told her BF and he comforted her (didn't tell the cops either, just kept it to themselves)
MC is unaware his GF is blackmailed, at the same time due to the shock of the rape develops a NTS fetish and tries getting into it with 2 unknown partners.
Kuroda the rapist by chance gets introduced by MC under the nickname of DARK as a the next partner, however MC notices he is THAT Kuroda guy MC is pissed but confused, however being the pushover MC is he lets him become a NTS partner, he plays her way too well but that's because he's been training her since the date of her rape under blackmail.
One thing to note is that during the NTS sessions he wears condoms but on the videos Kuroda sends (dating the time he raped Izumi) he goes bareback on her.
By the end in the "yesterday" video MC got from Kuroda he is lowkey telling Izumi to be his woman with sweet talking about his status, Izumi tells MC that she is refusing Kuroda's offer and will end that relationship with him today knowing Kuroda sent ALL of the videos to her BF, MC is ashamed of himself but also gets another video from Kuroda, impling he forced himself on Izumi again when she wanted to end it with him.

Seems the couple is on a Stand-off situation:
Kuroda is a prick who isn't willint to let Izumi go.
Izumi wants to end it but hasn't much power to get away from him, also blackmail.
Tarou is enjoying his fetish but also is conflicted about letting his fetish go while risking his GF's well being.

IMO this author rushed a story arc in one single chapter, so it feels messy.

He got the fetish because of that time she told him she got raped, she was desesperately calling Taro over and over the day after the rape so MC got anxious when he saw the large amount of missing calls he got from her.
