It's a huge implication from back in volume 2 that they are actively being sabotaged by Anti-Social organizations and the powers that be have way too much confidence in their skill. Plus, this is likely a case like Taimanin where we just see the worst cases and they probably as an organization have a much higher success rate

Yeah, there is that too, but that would only apply to joint ops like this one, or the one that ends Volume 3. Most of the joint ops we see are usually small scale where they get ganked, while this one is basically TPKs out the ass where they can't get the information from survivors since there are none.

Honestly the biggest in-universe flaw of the organizations is that they're too arrogant to change their ways, so even if they were to supposedly have a pretty decent survival rate, major failures would give no insight. We can also kind of see that they kind of have the Taimanin-view, of rather destroying the body of the failures than potentially letting them fall into the hands of another group.

To be fair, each season is its own continuity, with the semi-same cast of recurring characters. I can see why FNH does it; he might have his favourite archetype and/or it'd be hell to create a whole new cast of unique characters every time in a more linear narrative. It might just be that the clubs are having a bad week or something and their usual success rate is much higher, especially since antisocial organizations are actively aiding the monsters- I assume that every Taimabu season is the baddies finally getting their shit in line and blitzkrieging the human world. You could headcanon that after every season *someone* gets their head out of their ass and starts implementing reforms, but organizational change is not quite as exciting as watching hot babes get turned into seedbeds or whatever.
