As for the kneecaps, I think the artist is looking at real-life BBWs or BBWs in JAV and trying to create that same look in the knees. Which, Idk why he would want to as in anime/manga/hentai world you don't have operate in the realm of the real world and you can smooth out imperfections. Most of the time in anime/hentai medium you don't see the details of wrinkles on kneecaps or the back of elbows on girls because it's something that you can just make disappear and honestly even on the hottest RL girl it's not that great to look at. But up to the artist if that's what he wants to do.
