The artist is a little inconsistent about it, but from the chronologically first time we see him naked on page 6, it's clear that he had noticeable breasts and puffy nipples even before going through any special training, and they get even bigger in later scenes. It's a puzzling addition, since the doujin doesn't have a feminization theme or even mention his femininity. He's not even being made to take dubious drugs or anything; it's otherwise a mind break doujin with an infantilism subtheme.
