It is the Japanese idiomatic expression equivalent to English “It’s hard/tough to hear that”. So while the Japanese words do literally mean “My ears hurt”, the intended meaning is that it is hard/awkward for him to hear the (unpleasant) truth from her that he is the cause of her being a “bad girl”.
