I will be honest, I've kinda run out of steam doing these Macoto translations.
I'll likely won't do any more of their 'translations' unless they do something really special.

How do you extract text for translation. I'd do it myself, doctoring machine translation and fixing up the image if I had a decent way to actually get the raw text from the image.

I've found there really is no better way than to use use Optical Character Recognition software (or OCR). I use google lens for the job. It's not perfect, as sometimes picks up some Japanese onomatopes for dialogue, and order the sentence structure wrong, but it beats the old days of transcribing hiragana and kanji into google translate.
