I feel translating ”小宵は筋金入りのメスホモだった” as "Massive Homo" doesn't properly convey what's actually being said. The メス part is important. It's not just that he's gay but that he's also so dedicated to being feminine that he'd alter his body to be more appealing to men. Also, while 筋金入り could be translated as massive, hardcore or staunch are closer. That said, obviously "Koyoi has dedicated his life to the art of being a girly little faggot" wouldn't fit either. Personally I'd be tempted to put "Koyoi is a hardcore bitch", even though it's a departure from what's being said literally, as it conveys both that he's a jerk and jerk-off material. You could even restructure the comments earlier to reflect this, saying "This job can really bring the bitch out in you" and "Guess she's more than just a hardcore bitch after all"
