At first it was both of them fault, some arguing and Ntr guy took advantage of it(typical ntr setting) but later I would say it was more her fault, MC wanted to apologize but she didn't answer any of his calls, even more after the ntr guy told the mc what she did with him (what saved her was that everysingle moment thought of his "boyfriend").

I'm not sure either, maybe she wonders as family siblings or as family lovers. Of course, the answer is obvious given the oriented theme of this doujin and they went past the relationship as siblings since they had sex long ago.

The guilt comes from her feelings for her brother, and as why they didn't have sex, is obvious, both didn't have the guts to take the next step for their relationship.

That's why I think it's safe here both the main characters end up together, also the mc realized how much he loved her and how stupid himself was for not being honest earlier.

Page 59 makes it looked forced to me though. It was a challenge from her and he failed to please and make her fall for him. At the end she couldn't take her brother out of her mind ever.

Matsuri DIDN'T FELL FOR THE JERK. This is important, she used him to make herself forget about her feelings for her brother and it didn't work. And if you look at the jerk's POV he isn't really feeling it or more like he feels he can't fight against Matsuri's feelings for her brother, he even remembers her being happy when kissing her brother but conflicted when kissing him, he Fails and Matsuri only had Yuu on her mind even in the last round, he wasn't that stupid to not notice that, that's why he tried to piss the brother off, and still failed.

TLDR
Even Sex with the jerk isn't enough to cut her feelings for her brother. Her brother needs to man up (but that's another story), but in no way she will ever go back with the jerk, not here and not on any Mashiraga Aki's doujin from the future. I can bet on it and put my hands on fire because that's just his style just like you can tell the girl will fall for the jerk's cock on any story from Carn. Here the jerk never wins but greatly hurts the relationship between the main characters only to have them join together at the end, albeit a bitter sweet end.

Actually, the reason the sister keeps thinking about her brother is guilt.
She has never had sex with her brother before. She even says she's never let her brother do her yet in the first part. She's never even sucked him off even. Only thing we know is that the jerk has a bigger dick, so there is no comparison as to who is better at sex.
