Okay, clarification time:

Nyaruko is not Nyarlathotep, she is a Nyarlathotepian, in this, the various Lovecraftian horrors are their entire species, and none of them are as strong as the original versions, they ARE incredibly powerful by human standards(although both Mahiro and his mother can fight them off... with forks, it is outright stated that his family is special about this, being the descendents of an ancient deity hunter), but not on "reality warper" level like they are in the original books.

She IS incredibly old, so old that when Mahiro tried to search her age in a terminal that supposely held all the wisdom of the universe, it returned an error message, and very early in the series she was able to take 300 years of saved paid vacations off so she could stay on Earth, that means she is in the Planetary Defense Organization for at least 3600 years, she also outright states several times that she can shapeshift, so if this is her real form or not... it is heavily debatable, she claims it is, but she is also a notorious liar, it is worth to note that her brother took a form almost identical to depictions of Nyarlathotep once, when he was ready to fight, so make of that what you will, my personal theory is that they have two "true forms", a human-like "sleeper mode", and a monstrous "Full Force mode", and they can modify both forms at will

As for how the Lovecraft novels exist in this universe, I believe it was stated that Lovecraft had a Nyarlathotepian friend, and that he wrote the novels based on stories his friend told him, it isn't that he "exagerated", he simply took creative liberty with it.

Cuuko/Kuuko(also in this doujinshi) is a Cthughian, a species based on Cthugha the Living Flame, and therre is also Hasta/Hasuta(AKA Sir Not Appearing In This Doujinshi), who is a Hasturian, based on Hastur the Unspeakable One, there are even other species based on Lovecraftian horrors, like Luhy, who is a Cthulhi(this one is lifted straight from the books, there was no reason to create a species based on Cthulhu if there is one already), the Hodens, and other species which name aren't outright stated, but the ones I remember are Ato-ko, based on Atlach-Nacha, and Tsuruko, based on Ut'ulls-Hr'ehr.

About who Mahiro should choose... spoilers(this isn't shown in the anime, only in the Light Novel, which doesn't have an english translation): He chooses Nyaruko... only for her to do something in their wedding night that drains all his Sanity points, it is initially implied that it was something eldritch... but then the text all-but-states that it's simply that she is so ravenously hungry in bed that she may have proven more than he can handle, so he should be fine after she calms down, he gets a good night's rest and a bag of ice for his balls.
