Summary of the game: (may have some inaccuracies since I skimmed through parts and played it with a Machine TL):
The premise is already explained by Comrade Claus.

In the sister (Yoriko) route she's the only one the MC (Yuushi) sleeps with (there's actually very few choices and it's easy to get all the endings) and she gradually falls in love with him. At first she just did it to get back at her husband, but she realized that the MC was much better at sex than her husband (Mitsuo) ever was and she soon realized that she had feelings for him. That's why she called off having sex with him after the first few times since she feared that their relationship would change (after the scene ending at Image 68). She and MC didn't have sex for several days / weeks afterwards and he realized that she became more and more horny because of that. He felt her up a bit at breakfast then and brought her to the point that she would ask for his dick eventually. That was also the first time he made her say that she was his woman.

Soon after that he met her husband in a restaurant where he was hanging out with the woman he was cheating with (Kanade). He showed no remorse whatsoever and it was almost natural for him to keep doing this, even though his wife and daugter ran away from him because of his constant affairs. He asked how they were doing, but he wasn't too concerned and thought they would return to him eventually without putting in any effort himself.
The MC was enraged by this and this only strengthened his resolve to make his sister completely his. He reported to her that Mitsuo was still unfaithful and thus it would be okay if she kept cheating on him too. He also told her that she could get better at sex that way and more sexy in general which could win back her husband (that was an obvious lie, of course, but he said it to ease her mind).
The next sex scene after that is the Naked Apron one (113+) where she already really opened up to him and I think it was also the first scene where she said that she loves him and she doesn't care anymore if he cums inside or not (she feared that she could get pregnant before that). At the end of it she asks if she already became more sexy and the MC realizes that her husband isn't on her mind anymore. That scene was also witnessed by MC's niece (Midori) and she masturbated to it.

Later in the bath is one of the few choices that really matter. Yoriko got incredibly horny and asks her brother for sex. If you refuse, you get into the Midori / Niece route. If you comply, you get another sex scene with her where all of her worries about cheating and getting pregnant are gone and she already fell in love with the MC. Afterwards, there's a little exhibitionism scene in the park which makes her further lose her inhibitions. MC comes up with a plan to get back at her husband and she agrees to it.
A few months later her husband comes home to suddenly see that his wife returned. She gives him a drink to celebrate the reunion and only then he realizes that something is fishy. His daughter didn't come back with her and her belly got bigger. He loses consciousness then because his drink was spiked.
Once he wakes up bound to a chair, you see the scenes at image 167+. Yuushi and Yoriko happily perform fellatio on each other and she doesn't care about her husband waking up at all, instead telling MC how much she loves him and how splendid his dick is. Mitsuo throws a hissyfit and screams that they shall untie them, what he thinks he is doing, that Yoriko is his wife, how wrong all of this is, that they have to listen to him and blablabla. Eventually they go over to penetration and Yoriko says how much better her brother's dick feels compared to her husband and says that their love bore fruit with a child in her belly. She also tells Mitsuo off that it's his fault for constantly cheating on her and treating his family like shit. He pleads to start over and that he would even take care of her baby, but she says it's too late and that there's no way he can compare to her brother, both as a man and when it comes to sex. Mitsuo is completely devastated and Yuushi and Yoriko start another round of sex.
After a timeskip of a few months it's revealed that they completely cut ties with her husband and she's only formally married to him with the baby being registered as having Mitsuo as the father (because their brother / sister relationship is taboo). Yoriko happily awaits a second baby of the MC and they have pregnant sex again. Also, it's shown that Midori is jealous of them.

Niece (Midori) route (I will make this much shorter):
You get a few scenes where you see her with her boyfriend (Yuuma) who's basically a massive faggot. He turns down all of her advances for intimacy, even when she practically begs him to have sex and take her virginity. Her uncle notices his and tells her that he can help her out with her sexual frustrations. She agrees, but limits it to oral and anal sex at first. Her boyfriend keeps being a faggot and doesn't have sex with her, so she eventually allows Yuushi to take her vaginal virginity (320). Around this point, both of them get into SM and exhibitionism. On one of their walks outside together she has vibrators stuck in both of her holes and they meet Yuuma by coincidence. He doesn't notice anything at all, like the oblivious faggot he is, and she's basically over him at this point. Midori and Yuushi have a few more sex scenes. There's another choice near the end here where you can choose to pursue Kanade (the woman Mitsuo was cheating with) as well, which leads to the harem ending. If you don't, you get the Midori ending.
In it, both Mitsuo (who is a teacher at her school, btw) and Yuuma get letters to come to the AV room. Both notice that they haven't seen Midori for a while and suspect the other one of having fault for that. At that moment, a video starts where Yuushi and Midori are having sex with each other and she is calling him "Master". (388+) Yuushi addresses that they are producing this sex tape for her father and Ex-Boyfriend and she tells them that she is Yuushi's woman now. They blame Mitsuo for cheating on her mother and treating them like shit and Yuuma for being a massive faggot that never reciprocated her advances, even when she practically begged him for sex. Mitsuo throws a hissyfit again and blames everyone else, what they did with his daughter, what a faggot Yuuma is and what not, who takes it in and cries. I had a bit of sympathy for him, but not all that much. At the end of the video, both Mitsuo and Yuuma turned silent since they realized that they lost Midori.
After a timeskip, Yuushi and Midori happily continue their consensual SM relationship and Midori is pregnant with his baby. Also, Yoriko is jealous and wants to have sex with him, too.

Harem route: Yuushi invites Kanade into his house with the help of his sister and niece (who are both very much into him at this point), drugs her up a bit and "rapes" her. You can hardly call that rape, since she turns to love it within one sitting, saying that the sex is much better than any she had with Mitsuo.
Another timeskip and Yuushi is showing off his pregnant harem (I think to both Mitsuo and Yuuma), who are shocked, can't see their own incompetence, blablabla.
Epilogue: Yuushi is living together with all three women and their babies in his apartment and they all have sex again and are happy.
I didn't like the Harem route that much, since it felt rushed and it was a bit weird that all girls accepted this without any problems (maybe because he could satisfy all of them and didn't neglect them, unlike Mitsuo).

I really hate that there isn't a Netori tag. It's almost impossible to find stuff like this. Instead, all of it is crammed together under Netorare with ugly fat guys and the same plots over and over.
