The accreditations for this work are largely unknown.
The accreditations that we know are:
Yoshiyuki Sadamoto: The first 9 illustrations (pages 2-10), probably all Character Designs (pages 23-56), Background Design from pages 87, 96, 107-108, 129. Prop Design from pages 144, 152 -153, the whole Storyboard (except for the dogfight parts, those were done by Hideaki Anno).

You Yoshinari: Illustrations 11 and 15 (page 12, 16), the Rough Layouts on pages 321, 324 and 405 have entirely his style, while those on pages 362 and 384 have a partially similar style.

Masamune Shirow: The Mechanical Design of the first Aircraft (pages 57-72), the Background Design on page 88 resembles his work.

Miyatake Kazutaka: The Mechanical Design of the second Aircraft (pages 73-83), possibly the Background Designs of pages 90-94 as they have the same style and signature of the Studio NUE.

We know that the Color Boards (pages 420-613) were made by Art Directors Masanori Kikuchi and Hiroshi Sasaki as a competition.

It is possible to assume that the Art Directors may have done a large part of the Background Designs on pages 109-140 since some of those designs are on the Image Boards, in addition to the initial illustrations on the CD-ROM (pages 19, 20 , 21).
It is also possible that they are Rough Layouts cleaned up to that point or designs by studio animators, there are also some re-done. The one on page 116 has Hideaki Anno's signature and an "OK", so it was possibly approved by the director, there is also Anno's signature in the background on page 133-134, this time without an "OK" .

For this project Gainax hired many In-betweens and since You Yoshinari worked on the Rough Layouts, so many other In-betweens had to do it as well.

The script on pages 301-317 is by Hiroyuki Yamaga and corresponds to act 1/4.
