Poison is real female original in game

Censorship and controversy over their gender
The original Final Fight video game was versioned on the Super Nintendo consoles. An American who worked as a tester at Capcom reviewed the content during the process of locating one of the Japanese designers, and opposed that the protagonist of the video game hit women, referring to Poison and his partner Roxy, enemies to beat at the level 2 of the video game. However, Akira Yasuda had to point out that Poison and Roxy were "transsexuals". Despite their objections, Poison and Roxy were replaced by two punk-looking male characters named Billy and Sid. This change affected the American and European versions of the video game for Nintendo consoles, including the Game Boy Advance version, Virtual Console of the Wii and in the videogame Final Fight: Streetwise, which is included as an unlockable extra. The western versions of the Sega Mega-CD console have censored the characters in a different way, since Poison and Roxy appear but, because of their tiny garments that apparently inspired sex and lust, they put them long-sleeved shirts without cleavage and long pants. However, when the Super Nintendo version of Final Fight Guy arrived in the cartridge manual, Poison was sexually defined as "Newhalf" (transsexual). In summary, in the West and Japan, Poison is considered to be transsexual, but in the United States and Latin America she is considered a woman, although Capcom has not yet officially defined this issue. According to Akira Nishitashi in reference to this topic: "If it is a man or a woman, it is something that the players must judge as they wish to see it".

That was debunked years ago. The original concept sheet for Poison from when the game was called Street Fighter '89 even mentions that she's a "newhalf".
