It can be various reasons, so here's a few.

1) No one LOVES to get their heart broken/cheated on. A lot of people have experienced this. Most NTR fans suggest that they need to stop inserting themselves into the MC's shoes, and instead put themselves into shoes of the NTR guy. Then again, many people don't like the idea of inserting themselves into the point of view of, what's usually, a rapist.

2) Depends on the story. A good setup and plot with NTR in it can transcend a person's distaste in the fetish.

3) The suspension of disbelief is just too damn high. Fictional, fetish story or not, I'm supposed to believe a man who was formerly jailed for raping a woman when she was in high school, finds her years later and blackmails her with the EXACT same photo evidence used to get him jailed in the first place?!
That's not a joke. That was literally the plot of an NTR manga.
