Nice job. Here's a few comments, just picking nits and offering how I'd do things, I don't find them glaring:

(Thanks to the Dutch making an error similar to saying "yams" for "sweet potatoes" when first introducing guinea pigs to Nagasaki Japanese has two words derived from "marmot," マーモット and モルモット. The first means marmot, and the second means guinea pig. This is weird.)

p2: 闇町医者 stresses the fact that Dr. Takemi is unlicensed. It's sort of a portmanteau of 闇医者 (which means unlicensed doctor, think Black Jack) and 町医者 (a slightly archaic word for a doctor in private practice). So "unlicensed practice" or "underground clinic" or such is slightly preferable.
p2: 非合法な (higouhou na) just straight up means "illegal." I don't feel like putting "legal" in quotes quite captures the meaning. These aren't questionably (il)legal.
p5: 楽にする in a medical context means "give you relief" (from pain, matching the 辛そう in the previous thought bubble), so I would say more "I need to give him some relief," because I think that captures the particular connotation better than just "make comfortable," which doesn't necessarily imply cessation of pain. (could be "pain relief / relieve his pain," but I think explicitly stating the word "pain" eliminates the double entendre that should be preserved.)
p6: The だけ after the predicate in the lower right means that Dr. Takemi is saying "I'm only administering appropriate treatment as a doctor..." as in she's denying to herself that she has any sexual reason for touching his cock. I feel that's not quite the same impression I get from your translation ("must" would be more similar to べき, or a few other ways to say it), though other people may vary. (As a stylistic issue, I would generally say "in the most appropriate manner" and only say "in the manner most appropriate" if it were followed by a prepositional phrase it modified, like "in the manner most appropriate for the patient.")
p7: Feels like as a doctor and with her manner of speaking she'd use "ejaculate" for 射精 instead of "cum" (which works better as a translation for いく, used later when she's more aroused.)
私が触ってさらに興奮してる？ is "(Whoa,) He's getting even more aroused as I'm touching it?" (が binds 私 to 触って and indicates that she isn't the subject of 興奮してる, which is also what is modified by さらに) The next line is "There's already this much pre-cum coming out." p8: You missed the lower right panel. "I have to suck it all out"
p9: After the "That should calm him down a little" in the top panel, the middle panel is "he's *still* erect" (not *again* / back up as you have it, which would be また instead of まだ)
Also, the 辛そう in the lower right is again referring to "that looks painful/rough." (I kind of like using "hard" for a double entendre here, but there's a couple ways to translate it, including with different phrasing.) 〜そう is "seems" in the sense of emphasizing the evidence of the senses, not in the sense of "it seems to me" as in how you think, and thus *cannot* be used in the sense of "that's too bad (that I'm going to have to fuck my patient)," which is the connotation I get from your translation.
p14: I'd like to preserve the passive causative of 'But you made me inhale so much of your feminine musk"
p16: 頭が変になる should be something like "I'm going [to go] crazy" "I'm about to lose / go out of my mind" (other colloquialisms are fine too, just "my head feels strange" isn't natural English.)
p17: I like your translations here a lot
p21: I'd kind of prefer to keep the past tense, "I've been a pretty terrible adult as far as you're concerned, huh?" but yours is fine.
