I think the teacher is on to something... The "epidemic" is sexually transmitted. Pretty much the only one that survived is the black haired iinchou because she wasn't getting fucked after going inside the village, which is already infected at the point of sexual frenzy.
Also Hal placed some grey cloudy outlines on infected characters, which is seen at the hand on pg. 27 and the 4 characters in pg. 28.

I think what happened was a virus was created to help stem the falling birtth rate but it wasn't tested suffciently or got out before it was approved for human use. The virus mutated and everyone with the ability the ability to procreate is infected but the need is so deep and embeded in their breains that people over exshaust themselves breeding resutling in death. The girl that lived, she had a miscarriage which made it so she couldn't get pregnant again so she survived. She probably was raped before the incident and due to her miscariage she was immune since she couldn't have kids when the virus spread.

By that line it is fair to say there are other people that lived but can't procreate due to medical reasons or such so eventually humanity will die out.
