Combine rural communities, substandard education, and isolation; this is what you usually get. Alabama as a whole no, but plenty of famous real life examples in similar places such as Eastern Kentucky, Newfoundland, and Australia. I would say the South as a whole, barring Florida, parts of the Midwest, and West Virginia has this reputation.

Something to also consider is that 30~40% of reported sexual abuse is committed by a victim's family member. As much as porn/fiction can sometimes try to normalize it, the fact is that this fetish is not very vanilla. Especially out in the real world.
