I love how Kakuninii does this kind of mind control protag. There's a truth to Tenma's contradictions, even if some of the inconsistency is for comedy's sake. He's neurotic about what girls MAY think of him, but he's okay knowing for sure that Nao looks down on him. He doesn't even stop her from expressing it. On the date, he can play the slob without hesitation when, surely, strangers will judge him for it. What hurts him is the uncertainty, not knowing whether or not Kikyo's sick acceptance is real.

So while Tenma sounds flippant at the end, he's actually taken a big step and trusted a girl for the first time. He's putting faith that Kikyo's loving words and actions are true to her thoughts. And it hurts me how I know how hard that is. Of course, trusting someone is entirely different from KNOWING them. That difference is 28 stitches.
