That is not how it works. Wish-fulfillment revolves around situations that the audience wants for themselves and nobody (not even real life fat ugly rapists) wants to see themselves a fat ugly rapist. This is why 90% of all main characters in stories are either good-looking or talented or both (despite the fact that most of people in the world are not these things). The "evil pretty boy" is at least half-way and people would definitely rather identify with him than the fat ugly guy (unless fugly is absolutely amazing in other aspects). Also, there are quite a bit of people who thinks "evil pretty boy" is "cool and bad-ass."

The appeal of the fat ugly rapist type of hentai is how fucked up and taboo the situation is. Think rape, monster, NTR, bestiality, old men, etc. It has nothing to do with what the audience want for themselves except netori and certain rape stories.

Genres where the appeal is for the MC to represent the audience are the harem-type stories or even vanilla, where the MC is either average but everyone loves him for little reasons or he is a Marty Stu and everyone loves him. Think Sword Arts Online. Love them or hate them, who wouldn't want to be in their shoes?

It is entirely possible that the author meant for the fat ugly rapist to represent the audience in order to mock them but that is not the appeal of these type of stories.

Also, NTR and hentai stories where the story is told from the point of view of the character that is not meant to be the audience avatar is common. Take this story for example: it is told from the view of the ugly fat fuck but the "I" in the title is referring the boyfriend.
