a quick summary of the plot:

When the big queerrat sticks in it, she's surprised that it doesn't hurt and that she begins to like it a lot. After a couple months, she gets pregnant, and the queerrat tells her that they're going to have surgery so she can give birth faster. And Saki says ok as long as she gets to do it more.
