From what I could gather from context clues the dick is related to some sort of magical occurrence. Senjougahara appears to be entranced by it (notice the face she's making, and how desperate her actions seem, although it could just be general NTR-logic for all I know) and Kanbaru doesn't seem willing to stop her. Araragi, considering that he appears to be in a hurry, is rushing over to Kanbaru's home, likely because he knows about the situation to some degree, like he heard about it from Shinobu or something. He finally get's there, only to find that he's too late.

The faces and the content don't strike me as normal NTR stuff, it feels too much like a hypnosis sort of scenario, where either one or both of the characters aren't in their right minds. And it doesn't seem to be as focused so much on the whole breaking/cheating aspect of it, rather that this is just a thing that is happening that no one can control, etc.

To be fair I haven't read any of the author's other works so this could just be his style.

Nadeko gets mentioned several times despite not appearing. I think that the sudden appearance of Kanbaru's "snake" (Suruga Boa, as in Boa Constrictor) is connected to Nadeko's snakes, or at least Araragi thinks.
