But to answer your question, it's because everything is fetishized these days: whether it be violence, certain body parts, the idea of dominance,size, animals,smells and scents... Hell, I bet someone somewhere on this planet has a fetish for something as innocuous as opening doors. Point is, the sexual arousal/pleasure attained from the fetishized thing may be so great that traditional elements of sex (I.E. Penetration) might not even be depicted. So yeah.
