I just summary a little bit from note of Wilfriback:

The NTR had happen in this family as the old tradition. his father (not real father) is cuckolded by his grand-father (real father). As tradition, his grand-father only return the wife (MC's mon) when his father continue this tradtion. And then the story begins...

In the previous chapter his childhood friend who was accepted into MC's household was raped by his brother and passed around between him and his reak father.

She was broken and ended pregnant of his brother, typical netorare plot here.

The problem here (for me at least) is that MC found the oldman fucking his love on the previous chapter but seemingly forgot about that on this one, and lets not forget how she supposedly wanted help but for some reason kept her mouth shut but now adores the MC's brother to the point of bearing his child. => (Uploader's opinion: MC see his mother is fucked by his grand father in chapter 1 not his lover that's why he keeps his secret)

So much reveals made MC to freak the hell out and lose his sanity as he comes to a realization as he laughes like a madman. Meanwhile, MC's real father finally passed MC's mother to his brother and they had sex which is something he was looking forward for a good time, years. I guess he got broken too in the past and went insane.

Time skips and he seems fine, he gets along with his childhood friend who have birth to his brother's child and both apparently act like a formal couple while his brother married his mother (seems his real father passed away), his brother fucks both his mother and childhood friend as he endures it watching.

Time skips again and last page is the cycle repeating itself but this time it's MC turn.]

Just a fix there. The one MC found on the first chapter not his love being fucked but his mother Tomoe doing it with his grandfather (real father), so that solved how he never found out about his childhood friend until second chapter. Same face/body can be misleading if you don't read it.

1) its funny to me how everyone assumes the guy is the mc, in most ntr manga the focus is almost always on the woman making the osananajimi the true mc.

2)like most people here I like it when things sometimes deviate from the genre formula, so I was kinda hoping when the bf ran away he was running to board the house up and set fire to it with everyone in it(but then this isn't hal, DaHooch, or any of the mangaka from Comic Mate....).

Actually, in the first few pages you could notice that there's another woman there standing besides MC's half-brother who looks older than MC's childhood friend. She also has a slightly different hairstyle(no braided ponytails); that's how I recognized it's the 2nd woman whom the fat old man was fucking.
